Why waist to height beats BMI for risk
Waist to height ratio divides your waist circumference by your height. The widely used threshold is 0.5: keep your waist under half your height. It is simple, needs only a tape measure, and works across most adult populations without separate cut-offs for men and women.
What makes it more informative than BMI is that it measures where fat sits rather than how much you weigh. Visceral fat, stored around the abdominal organs, is the fat most strongly associated with metabolic risk in population research. BMI cannot see it, which is why a muscular person and someone with high visceral fat can share a BMI and be in very different situations.
The UK National Institute for Health and Care Excellence recommends using waist to height ratio alongside BMI for exactly this reason, and several health bodies have moved in the same direction.
The calculation and the thresholds
Example: 85 cm waist, 175 cm height
85 / 175 = 0.49
| Ratio | Category | Interpretation |
|---|---|---|
| Below 0.40 | Below healthy range | May warrant assessment for underweight |
| 0.40 to 0.49 | Healthy | The commonly used target zone |
| 0.50 to 0.59 | Increased central adiposity | Raised metabolic risk in population data |
| 0.60 and above | High | Substantially raised risk |
Healthy waist ceiling by height
| Height | Keep waist under |
|---|---|
| 155 cm | 77 cm |
| 160 cm | 80 cm |
| 165 cm | 82 cm |
| 170 cm | 85 cm |
| 175 cm | 87 cm |
| 180 cm | 90 cm |
| 185 cm | 92 cm |
| 190 cm | 95 cm |
Why fat location matters more than fat quantity
Visceral fat surrounds the abdominal organs and is metabolically active, releasing free fatty acids and inflammatory signals directly into the portal circulation. Subcutaneous fat, stored under the skin on hips and thighs, behaves differently and carries a weaker association with metabolic disease. Waist circumference is a practical proxy for the first kind.
This is the specific blind spot in BMI. Two people at a BMI of 27 can have very different waist measurements, and the research consistently finds the waist figure more predictive of cardiometabolic outcomes. Neither measure is a diagnosis, and both are screening signals rather than verdicts.
How the measures compare
| Measure | Captures | Misses |
|---|---|---|
| BMI | Overall size relative to height | Muscle vs fat, fat location |
| Waist circumference | Central adiposity | Does not adjust for height |
| Waist to height | Central adiposity adjusted for frame | Muscle mass |
| Body fat percentage | Total fat vs lean mass | Where the fat sits |
Using waist to height alongside body fat percentage covers most of what a single measure misses.
Measuring consistently
- Measure directly against skin, not over clothing.
- Find the midpoint between the lowest rib and the top of the hip bone.
- Stand relaxed, arms at your sides, at the end of a normal breath out. Do not hold your stomach in.
- Keep the tape level all the way round and snug without compressing.
- Measure at the same time of day, ideally morning before eating, and track the trend over weeks.
